how to tell what size A,B,C,D?

On my 99 EC200 it is marked on the back of the cylinder, just about the reed cage.

I would assume that if your asking, then yours is not marked?

Next time your due for a topend, remove the cylinder and have it measured a good shop can do that for you. I think it is check in 3 different locations of the cylinder. Based upon those numbers then you can check your piston options.

I looked all over the back and there is no mark, i was searching wiseco pistons and to me it seems like they only come in one size for my 2005 200. so does the size really matter?
 
To answer your question, yes it matters. If it is too loose it will not perform as well as it can. If it is too tight you could have problems with seizing. Since it is a 2005 and you have it apart, I assume it has some miles on it and needs a rebuild. If you can feel any ridge at the top of the cylinder, I would get it diamond honed and measured so you don't waste money, time and HP.
